Medicare

Medicare is our country’s health insurance program for people age 65 or older. The program helps with the cost of health care, but it does not cover all medical expenses or the cost of most long-term care.  Medicare is financed by a portion of the payroll taxes paid by workers and their employers. Medicare Part A is hospital insurance that helps cover primarily inpatient care in hospitals.

  • Inpatient care in a skilled nursing facility (not custodial or long term care)
  • Hospice care services
  • Home health care services
  • Inpatient care in a Religious Non-medical Health Care Institution

With Medicare Part A alone and no supplemental insurance you will pay a $1132 deductible for hospitalizations, the cost of the first three pints of blood (some places over $300 a pint), $283 per day for days 61-90 and $566 per day for the life-time reserve days 91-150 , and total cost for day 151 and on (2011). For skilled nursing facility care you will pay $141.50 per day for days 21-100 and total cost for days 101 and on.

Medicare Part B is medical insurance that helps pay for doctors’ services, some preventive services, and many other medical services and supplies that are not covered by hospital insurance. If you have Part B, you pay between $96 and $115 premium each month that comes out of your Social Security benefits (this depends on your income) .

In general, Medicare Part B covers:

  • Medically-necessary services — Services or supplies that are needed to diagnose or treat your medical condition and that meet accepted standards of medical practice.
  • Preventive services — Health care to prevent illness (like the flu) or detect it at an early stage  when treatment is most likely to work best.

With Medicare Part B alone and no supplemental insurance you will pay 20% of all outpatient expenses, doctor’s visits, medical expenses, ambulance service, and medical equipment cost.  You will also pay any excess charges that can be 15% above the total bill (2011).

It is important for you to understand that Medicare does not cover everything, and it does not pay the total cost for most services or supplies that are covered.  If you end up in the hospital or in need of a doctor’s care you will have out of pocket expenses that for many could be very costly.  But don’t worry.   • Apply early. It generally takes at least two weeks for a supplement application to be processed.  If you are turning 65 be aware that some insurance companies will take applications six months from your birthday and all will take applications three months out.
  • Answer the questions truthfully. If you have to answer health questions on the Medicare supplement application, answer them truthfully. The company checks your medical records, anyway. With few exceptions, when you apply before six months past your birth month, you cannot be denied coverage and there are very few medical questions to answer.
  • Keep a copy for yourself. Whenever possible, keep a copy of your application for yourself. This always helps if there is a problem or question about something on the application.
  • Ask questions upfront. It is always best to get all of your questions answered prior to applying for the policy.
  • Remember to cancel your old policy. If you are replacing an older Medicare Supplement or Medicare Advantage plan, you must cancel your old policy effective the day your new one starts.  This keeps you from having double coverage and double payments with no added benefits.  Always wait until your new policy is approved before cancelling an old one.

Texas Medicare falls into the Medicare coverage Americans receive once they reach the age of 65.  Texas Medicare is broken up into four parts: Par A, Part B, Part D and Supplements.  Understanding each part will help you choose the best options for your coverage.

Texas MedicareTexas Medicare Part A

Medicare Part A is used mostly for in patient hospital coverage (you were admitted by a doctor), Skilled Nursing Care and hospice care.  Part A will automatically enroll most people and there are not premiums (monthly costs) involved in Part A for the vast majority of people who are turning 65.  Medicare Part A does have a deductible and co-insurance.  To understand exactly what this will cost you you first need to understand the Texas Medicare part A benefit period.  The benefit period is pretty easy.  It is any instance that falls into the Part A coverage and the period of 63 days after you leave the hospital.  For example, if you were admitted to the hospital and left seven days later.  If you were to go back into the hospital 40 days later this would still be covered under that same benefit period.  If you had to go back in 64 days later it would open a new benefit period.  This is important because the deductible is tied to benefit periods not to an annual deductible, which is what most people are used to.

Texas Medicare Part A – What You Pay

Now that you understand the benefit period we can get a picture of what it will cost if you only take regular Medicare.  For each benefit period your costs are:

Hospital Visits

  • Deductible of $1,156 and no additional costs for the first 60 days of your hospital stay as of January 2012
  • Days 61-90 will cost $283 per day
  • Days 91-155 will cost $566 per day
  • After day 155 you are responsible for all costs
Skilled Nursing Care
  • No charge for Days 1-20
  • Days 21-100 are $141.50 per day
  • After Day 100 you are responsible for all costs
There are minimal costs for Hospice and other minor services offered by Part A that you can find over at the hard to read medicare site

Texas Medicare Part B

Part B was designed to help you with outpatient services.  So for emergency visits to the hospital where you are not fully admitted or just a visit to your local doctors office this is where Part B will take care of you.  Texas Medicare Part B has three costs associated with it; premiums, a deductible and coinsurance.  The nice par about Part B is that it that the charges are not nearly as complicated as Part A.  The costs involved with your Part B coverage are:

  • Premium of $99.90 per month as of January 2012
  • The annual deductible is $140
  • The coinsurance is 20%
That is all well and good but lets use an example.  If you went into your local doctors office and had outpatient surgery that the total cost was $1000 what would you pay during the month that you had that procedure?
  • Your Texas Medicare Part B premium of $99.90 (you pay this whether you use the service or not
  • The deductible of $140
  • 20% of the remaining charges or $172 ($1000 – 140 = $860 * .20 = $172)
  • Total of $411.90
If you had any other outpatient services then you would not have to pay the deductible again it would only be the 20% coinsurance and the monthly premium.

Texas Medicare Prescription Drug Programs

During the early part of the 2000′s congress passed legislation that created prescription drug coverage for seniors to help with the rising costs of prescription drug coverage.  These plans are managed by private insurance companies and are highly customized to the prescriptions that you personally take.  Currently the average for plans across the country is $31.18 per month.  You are eligible for a prescription drug program when you are enrolled for Part B.

There is a penalty if you do not enroll once you have Part B coverage.  It is a pretty easy formula of 1% per month of the national base premium for every month that a person is not covered by a creditable PDP (prescription drug program).  To get this into something manageable lets look at an example:

  • John Doe is enrolled in Part B in January 2011 but does not get PDP until January 2012
  • So the penalty would be 1% of $31.18 times 12 months or $3.74
  • You would pay $3.74 per month extra on your PDP premium
